Sinopsis

 
~ Eiji Hino adalah seorang pria tanpa mimpi, pekerjaan maupun keluarga. Di tempat lain, sekumpulan monster yang menamakan diri mereka Greed terbangun setelah 800 tahun dan menyerang manusia demi memenuhi kebutuhan mereka. Makhluk tersebut terbagi ke dalam 5 jenis dengan bawahan yang disebut Yummy. Setiap Greed dan Yummy kehilangan bagian dari tubuh mereka, namun apabila mereka mengonsumsi hasrat manusia yang cukup, akan terjadi regenerasi pada tubuh mereka di samping mereka mendapatkan kemampuan baru dan apa yang disebut dengan Medal.

Greed dari jenis unggas bernama Ankh malah membelot dan memberikan Eiji sebuah sabuk (ŌOO Driver) dan 3 Medal (OOO Medal) untuk bertarung melawan Greed dengan wujud sebagai Kamen Rider OOO untuk menjadi raja dari Greed itu sendiri. Kemudian muncul organisasi misterius bernama Kogami Foundation yang berniat membantu Eiji dalam setiap pertarungan melawan Greed, walaupun tidak diketahui apa motif sebenarnya dari bantuan tersebut.

Seperti halnya Greed ketika membunuh manusia, saat Kamen Rider OOO mengalahkan Greed atau Yummy, Eiji akan memperoleh Medal dan kemampuan baru ~
